NATIONAL AUDIT PROJECT 4 - FAQs

We have listed here a few question frequently asked by local reporters.

Do I have to fill the form all at once?

Since it is intended to get detailed information leading to the airway incident the NAP 4 data collection forms are quite extensive. We have tried to make this as user friendly as possible.

• You can save the form at any stage and come back later
• Upon logging in you will be taken to the last page you were working on.
• Please remember to logout when you have finished
• It will be easier to collect data if you take a print out of the downloadable versions of the form with you.

I have filled in all the details. How do I submit the form and what happens after I submit ?

In the last page of the form you have the option to submit the form. Once you have submitted you will not be able to edit the form. However the data will be reviewed by an administrator from DAS and if there are any doubts you will be contacted again with instructions about how to login again.

What do I do in case I forget my password ?
